Purpose: Take Charge of your Health! A Cancer Survivorship Forum is a FREE educational program intended to encourage and empower cancer survivors, families and supporters. Participants focus on taking charge of health matters concerning cancer survivorship issues. We believe by seeking information, accessing resources and effectively communicating needs to a care team for support, aim to provide inspiration for improving quality of life. Healthcare Professionals, Community and Faith-Based Organization members and under-represented populations are also encouraged to attend. A light breakfast and healthy lunch will be supplied.
